The mission of Chapa-De Indian Health is to advance the health and well-being of American Indians and low income individuals living in our communities by providing convenient access to high-quality, compassionate care.

Letter from Lisa

Dear Friends and Supporters, 

 As we reflect on the past year, I am filled with gratitude and pride for the progress and resilience demonstrated by the Chapa-De Indian Health team, our patients, and our community. Together, we have navigated challenges, celebrated achievements, and continued our mission of providing high-quality, compassionate healthcare to those we serve. 

This year holds special significance as we come off our 50th anniversary, a milestone that allowed us to celebrate our history while looking forward to the future with renewed determination.

As we enter 2025, we are proud to continue enhancing our healthcare services, expanding our remarkable team, and advancing plans for our new South Placer Health Center in Rocklin.  

I am delighted to highlight many of these exciting plans and achievements in this annual report.

Quality/HEDIS Results

Chapa-De is committed to providing quality healthcare services. We track a variety of data metrics to help us understand how we are performing across our health centers. Our performance results are based on the Healthcare Effectiveness Data and Information Set (HEDIS). These results show how Chapa-De compares to other healthcare providers.

 

90th Percentile (we excel and are in the top 10% of all providers):

Highlighted Events

Our quality scores reflect the level of care delivered in daily patient interaction. We also host special events each year to reach additional patients and to offer services that are not regularly available.

 

Colon Cancer Screening Initiative

1,308 Chapa-De patients received no-cost at-home Cologuard colorectal cancer screening kits in 2024 with support from Partnership Healthplan and Exact Sciences. These kits were mailed directly to patients and gave them the option to complete a non-invasive screening to check for signs of cancer in the privacy of their home. We were pleased that 31% of patients who received a kit completed the screening. This is one more tool to help our community to be proactive about their health and to catch signs of cancer early when it can be treated.

Colorectal Cancer Screening amongst Chapa-De patients enrolled with Partnership Healthplan reached 44.64% (thanks to the Cologuard project). Compared to, overall, amongst all Chapa-De patients, we achieved 38.4%, and the national 50th percentile benchmark is 39.81%.

 

HPV Vaccination Improvement Project

Youth who receive two doses of HPV vaccine are less likely to develop some cancers later in life. In 2024, with a focused effort on educating about and improving HPV vaccination rates in partnership with the American Cancer Society, we achieved a notable increase in the number of male and female youth who completed the HPV vaccine series by the recommended age of 13. It is important to complete the vaccine series by this age to gain the most protection against human papillomavirus and associated preventable cancers.

Expansion of Specialist Services

Chapa-De has expanded our direct contracts with specialists to reduce care delays and offer streamlined access to high-quality specialist services. In 2024, we finalized partnerships that make crucial cardiology and physical therapy services available to Chapa-De patients, without having to be referred out.

Harnessing Technology to Enhance Communication & Care

AI Scribe

In April, Chapa-De began implementing DAX Copilot in our medical department. This exciting new technology is an Artificial Intelligence-powered tool that automatically documents patient encounters, allowing our providers to focus more on patient care and less on administrative tasks. By the end of the year, ten primary care providers and one psychologist were consistently using the technology. By integrating DAX Copilot, Chapa-De aims to improve provider satisfaction and enhance patient experiences by embracing technological advancements. 

Nurse Practitioner Adam Borruso says, “It is saving me time and mental space to be able to focus on patients and coordination of care. Honestly, it’s fantastic and I cannot imagine care without it in the future.”

Enhanced Patient Communication Options

Throughout 2024, Chapa-De focused on improving patient communication experiences. We recognized that callers were waiting too long on the phone and that many patients prefer to communicate and schedule appointments electronically. We focused on adding staff to our call center and promoted using online tools to enhance communication and access to health information. The patient portal and Healow app are online tools that allow patients to manage their healthcare conveniently online. Patients can schedule appointments, ask questions, and refill medications at any time of the day without calling in. They can also view personal records, track referrals, and more.

Resident Physicians at Chapa-De

In July 2024, Chapa-De welcomed the first class of resident physicians to our team through a new partnership between Common Spirit/Dignity Health, Sierra Nevada Memorial Hospital and Chapa-De. Two residents per year are welcomed into the 3-year Sierra Nevada Family Residency Program. The first year is spent predominately practicing hospital medicine in Sacramento. The second and third years are focused on delivering care in the Grass Valley community – splitting time between Chapa-De and Sierra Nevada Memorial Hospital. 

The first two residents, Dr. White and Dr. Sparr, began seeing patients regularly at our Grass Valley Health Center in July. Drs. White and Sparr provided care to 315 Chapa-De patients during 462 appointments in 2024. The second class of resident physicians, Dr. Camille DeRome Polik and Dr. Karli Haugen are completing their intern year now and will join the Chapa-De team and Grass Valley community in July 2025. 

Being a part of a medical residency training program is an exciting opportunity for our patients, our staff, and the wider community. Chapa-De is proud to offer this rare training opportunity within a tribal health facility. We are fortunate to welcome this next generation of doctors to our growing team and are excited to share our knowledge and perspectives on delivering effective and culturally informed care in a rural setting.

New Enhanced Care Management Services

Chapa-De has added Enhanced Care Management as part of our Care Coordination Program. This new benefit, covered by Medi-Cal, is designed to provide more intensive assistance to patients who are at the highest risk of facing urgent health challenges or hospitalization due to complex medical and social needs. 

In May 2024, Chapa-De welcomed its first Enhanced Care Coordinator, Henry Griep. Enhanced Care Coordinators play a vital role in advocating for and supporting community members to access the clinical and non-clinical services they require. Henry specializes in coordinating physical, mental, dental, and social services for clients. Henry is dedicated to meeting patients where they are to assist them to manage healthcare services and to connect them to additional health-related resources based on their unique needs.

In 2024, 549 individuals received assistance, resources and navigation from the Chapa-De Care Coordination Department. XX patients received Enhanced Care Coordination Services.

Excellence Spotlight: Clinical Pharmacy Program

The Chapa-De Clinical Pharmacy Program had an exciting year of growth and recognition in 2024. Chapa-De is honored to have received recognition from the IHS National Pharmacy Council in August for our Asthma/COPD program led by our Clinical Pharmacy team. This program continues to grow, with 98 one-on-one visits in 2024 dedicated to educating patients on proper inhaler techniques, improving self-management, and optimizing asthma treatment. Patients who had frequent inhaler refills at the pharmacy or who received a referral from their provider were invited to join this program and try the “In-Check Dial Tool” to test their inhalation technique. The In-Check Dial Tool revealed that over 90% of patients use incorrect inhalation techniques. Targeted interventions through the Asthma/COPD program have led to significant improvements in patient outcomes.

In 2024, our Clinical Pharmacists also supported 13 patients to be cured of Hepatitis C and they regularly meet with patients who take multiple medications to ensure their regimens are personally tailored to prevent drug interactions and optimize benefits. 

Pharmacist-led hypertension clinics continue to support patients to better control their blood pressure. During the year, our Clinical Pharmacists completed 481 hypertension visits with patients and they continue to receive a steady flow of referrals. Additionally, the pharmacy is proud to provide American Heart Association-validated blood pressure cuffs to patients enrolled in Partnership Health Plan.

Chapa-De’s new GLP-1 workshop offers patients valuable guidance on how to properly use GLP-1 injectables and enhance their treatment through lifestyle changes. Upon completing the course, Chapa-De’s pharmacist monitors the medication’s effectiveness and has the authority to adjust the dosage as necessary. Many patients using these medications to lose weight have already reached their weight goals and are now considering the possibility of gradually reducing their GLP-1 usage. Between May – December 2024, we’ve successfully educated 128 patients on the benefits and proper use of GLP-1 medications.

50th Anniversary Celebrations

On May 14, 2024, Chapa-De staff gathered at @The Grounds to celebrate 50 years of advancing the health of our communities. The all-staff event featured insightful guest speakers, including members of the Board of Directors, who reflected on Chapa-De’s origins and growth. A three-part short film series premiered, highlighting local elders’ stories about the organization’s early days. The event also included cultural activities such as Indian Tacos, crafts, and performances featuring native flute, drumming, and dancing.

On September 7th, 2024, Chapa-De hosted a special Open House event at our Auburn Health Center to mark our 50th anniversary with the community. The event brought friends, colleagues, and community members together to share stories of Chapa-De over the years and to celebrate our many successes. 

Attendees enjoyed crafts, a women’s drumming circle with Mother Drum Ishka, and a performance by two-time Grammy Award-winning Native American flute artist Mary Youngblood. The celebration also featured the premiere of three videos recounting Chapa-De’s history and showcased services from departments like Diabetes, Maternal Child Health, Care Coordination and Substance Use Disorder.

Dedication to Culturally Relevant Care

On October 8, 2024, staff participated in a transformative meeting led by facilitators from the Native Wellness Institute, Wade McGee and Shalene Joseph. The session emphasized being allies within the native community and highlighted personal accountability and positive energy in shared spaces. Staff demonstrated openness, curiosity, and dedication to Chapa-De’s mission of compassionate and culturally relevant care. Together, we participated in bonding activities, shared personal experiences, and worked to bring what we learned to our everyday care practices.

Local Elders Honored and Celebrated

Our annual Elder’s Holiday Luncheon on December 7, 2024, brought 189 attendees together at The Ridge in Auburn. The event honored Chapa-De’s 50th anniversary with a commemorative slideshow featuring historic photos from Chapa-De’s history and those who made it a possibility. Wanda Enos Bachelor offered a blessing, and participants enjoyed a buffet luncheon, prize drawings, community acknowledgments, and appreciation gifts. Organized and staffed by Chapa-De employees, the luncheon upheld a long-standing tradition of recognizing and appreciating local elders while supporting a valued local business.

$7,000 Awarded to Local Students

Chapa-De Indian Health awarded seven $1,000 scholarships as part of the April Moore Memorial Scholarship program in 2024 to Native American students in our region. These grants supported their continued higher education goals.

April Moore was a Member of Chapa-De’s Board of Directors for 26 years and served as the Board Chair from 2012-2015. She spent her life working to advance the American Indian community and helped to make Chapa-De what we are today. April passed away in 2015 but her impact in the native community continues today.

Since its inception in 2016, Chapa-De has awarded over $25,000 to local students (from or attending school in Placer, Nevada, Sierra, Sacramento, or El Dorado County in California) through this program.

Cultural Training Highlights

2023 Cultural Training Highlights

As a Native American health center, we are committed to continuing investments in culturally competent care through team member trainings and programs. In 2023, we hosted two all-staff events focused on cultural trainings. In May, we hosted the United Auburn Indian Community Preservation Team to focus on teachings related to Nisenan Life. This training is relevant because we are on the traditional lands of the Nisenan, Miwok and Maidu, past and present, and honor with gratitude the land itself and the Nisenan, Miwok and Maidu people.

 

In October, we partnered with Sierra Native Alliance to host Indigenous Wellness: Natural Healing & Resilience Teachings and Workshops. As part of this event, Chapa-De employees learned about native healing practices and were engaged in the following natural healing and native crafting workshops.

Meet our 2024 Scholars:

Corey Ellis

 

completed course work at Sierra College with plans to transfer to UCLA to pursue an undergraduate degree in English. He is currently writing his first novel that details his family’s history living in Nevada County for five generations. Upon graduating from college, he looks forward to returning to his hometown of Grass Valley with not only a degree but a broadened view of the world.

Isabella Nguyen

 

is studying Anthropology at Portland State University. Her life goal is to serve, empower, and motivate American Indian youth and families navigating the challenges of language loss and underrepresentation within their tribe. She aspires to work in the film industry while remaining closely connected to tribal communities to tell authentic stories that will help amplify the voices and experiences of Indigenous peoples.

Molly Briggs

attends Baylor University, majoring in Business Finance/Accounting and minoring in Economics. After graduating from college, Molly hopes to work in investment banking. She also hopes to raise awareness of what it means to be American Indian today and support other native students who are striving for a college education.

Zachary Hatten

 

was accepted into the Sierra College and Sacramento State bachelor’s merger program where he will obtain his bachelor’s degree just 6 months after graduating from Sierra College with an associate’s degree and an RN license. He plans on becoming a Registered Nurse in either the Emergency or ICU departments. As a descendant of the Karuk Tribe, he hopes to help people in times of need, especially those in the Native community.

Zoey Hatten

 

attends Sierra College with hopes of one day entering the medical field. Her career ambitions include becoming a registered nurse, inspired by both her grandmother and her brother. Zoey plans to use her education and career to give back to the American Indian community by sharing all the skills and knowledge she has acquired throughout her life. 

Zienna Hatten

 

attends Boise State University, pursuing a Criminal Justice major. It is her passion to learn more about law and order and how the judicial system works and how it can be improved. Her ultimate career goal is to become an FBI agent. She hopes to use all the knowledge she obtains to give back to American Indian communities.

Andres Ramos

 

is working toward his Doctor of Medicine (MD) at UC Davis School of Medicine. His career ambition is to specialize in orthopedic surgery. He plans to practice medicine that is tailored to the cultural benefits and practices of American Indian patients, as well as addresses other social determinants of health that negatively affect American Indian communities.

South Placer Health Center Progress

South Placer Campus

In 2024, Chapa-De made significant strides toward developing our new health center in South Placer County which will be located at 1451 West Stanford Ranch Road in Rocklin. The 62,193-square-foot facility, designed for future growth, is anticipated to open in late 2026 and will initially provide medical, dental, behavioral health, and diabetes care services.

 

A blessing ceremony in March marked an important milestone as the project advanced through key permitting, planning, and environmental requirements. With approvals secured, construction is set to begin in 2025. The site design embraces a healing, nature-focused environment with walking paths, benches, and green space connections. The building’s exterior will complement its natural surroundings.

Chapa-De engaged a muralist to design a large mural featuring a Native American theme, which will be a focal point of the health center. Additionally, three construction interns from the United Auburn Indian Community (UAIC) are gaining hands-on experience under the mentorship of Construction Manager Jack Williams, exemplifying Chapa-De’s commitment to advancing American Indian well-being.

 

The new clinic will provide a more convenient option for patients currently traveling from South Placer and Sacramento counties to our northern locations. It will also address the needs of thousands of Medi-Cal members in Rocklin, Roseville, and Lincoln who currently lack consistent access to primary care services.

 

This thoughtfully designed facility will meet the community’s evolving healthcare needs while staying true to Chapa-De’s mission of compassionate and culturally relevant care.

Steps to Maintain Quality of Care Amid Growth

To support Chapa-De’s continued expansion, we are proactively restructuring key departments and creating new roles to sustain our happy workforce and high standard of patient care. In 2024, we engaged the national healthcare consulting firm Gallagher to assess staffing needs and organizational growth strategies. Their recommendations emphasized maintaining Chapa-De’s culture of personal connection and exceptional patient care as we expand. With this in mind, we are growing our team and enhancing development opportunities for our current team members.

New Learning & Development Department

 

Chapa-De’s ongoing commitment to staff development is a critical part of our growth strategy. With the introduction of new training and advancement initiatives, we are fostering a culture of learning to help employees adapt to evolving technologies and processes. In 2024, 110 different voluntary on-demand courses were made available through the training program and staff completed 220 hours of training. These opportunities are designed to empower staff with the skills needed to deliver exceptional care and support innovation within their roles. Through thoughtful professional development, we are equipping our teams to meet the demands of a rapidly changing healthcare landscape.

Workforce Development Initiative

 

To ensure proper support for staff and effective leadership, structural updates are underway. We are recruiting for leadership roles in key departments, including IT, Risk Management, and Project Management. These efforts, along with planned departmental assessments, are part of a multi-phase workforce development strategy designed to maintain seamless operations and high-quality patient services across all Chapa-De locations, including the upcoming South Placer clinic.

Year 1

 

  • The “first year” resident physicians have been at Methodist Hospital in Sacramento. They have been gaining extensive experience with delivering babies, emergency care, and hospitalizations.

Year 2

 

  • The “first year” resident physicians will offer primary care services at Chapa-De Indian Health and hospital based services at Sierra Nevada Memorial Hospital. The residents will spend two years offering these services in Grass Valley.

 

  • As the “first year” residents move into family medicine training, the “second year” of resident physicians will join Methodist Hospital for their first year of the program.

Year 3

 

  • The original “first year” resident physicians will complete their final year of residency at Chapa-De and Sierra Nevada Memorial Hospital. Upon completion, they will be eligible to receive board certification and to practice without supervision.

 

  • The “second year” of resident physicians will join Chapa-De and Sierra Nevada Memorial Hospital for two years.

 

  • A new set (2) of residents physicians will join Methodist Hospital in Sacramento.

The Chapa-De South Placer Health Center will serve over 15,000 patients at full operations. We anticipate the clinic’s opening in late 2026 and are committed to ensuring its success while also maintaining appropriate support and staffing at our current sites. To achieve this, we’re planning a phased approach for our staffing expansion.

 

Phase 1: Establishing New Roles & Departments

In this initial phase, starting in early 2025, Chapa-De will be adding four new positions to lay a solid foundation for the expansion.
